AMENDMENT OF



PROFESSIONAL SERVICES CONTRACT



THIS AGREEMENT made as of the (34 _ day of Ot}oboey _, 2016 in the Territory

of the Virgin Islands, by and between the GOVERNMENT of the Virgin Islands

DEPARTMENT OF PROPERTY AND PROCUREMENT, on behalf of the DEPARTMENT

OF HEALTH, (hereinafter referred to as “GOVERNMENT”) and CLEARWATER

RESEARCH, INC. (hereinafter referred to as “CONTRACTOR”).



Both parties witness to being duly authorized to execute this agreement and will attest

accordingly when and where necessary.



WITNESSETH



WHEREAS, pursuant to Contract No. POO8DOHC16 approved by the Governor on January

26, 2016 and its Exercise of Renewal Option, approved by the Commissioner, Department of

Property and Procurement on April 12, 2016 (which constitute and are hereinafter referred to as

the “Contract”), the GOVERNMENT contracted with CONTRACTOR to provide services

conducting a health survey; and



WHEREAS, the parties desire to amend the Contract to increase compensation by $35,250 to

a total of $101,144; and



WHEREAS, in order to meet the data collection needs of the Territory, the Scope of Work

shall be amended to read “Cell phone interviews shall constitute 60% of all interviews conducted

after June 2016”; and



WHEREAS, the number of interviews shall be amended to read “Not to exceed 1440 complete

interviews in 2016”; and



WHEREAS, the amendment of the Contract pursuant to this Amendment of Professional

Services Contract shall not result in any change to the scope of work provided for under the

Contract other than the aforementioned changes in percentages of cell phone interviews and total

number of complete interviews for 2016.



NOW THEREFORE, for mutual and valuable consideration, the receipt and sufficiency of

which is hereby acknowledged, the parties hereto agree as follows:
