Dear Mr. Rotger: 
 
The application for the above-referenced CZM Major Permit has been reviewed and found to be 
INCOMPLETE. Please see attached list of deficiencies. All responses should correspond to deficiency 
items.  Responses will be accepted within ninety (90) days of this notice of deficiency unless a request 
for extension is granted. See 12 V.I.R.R. § 910-7. 
 
 
 
 
 
 
 
All items noted on this checklist, the application, the drawing requirements handout, and in the EAR 
guidelines, as applicable, must be included or adequately addressed for an application to be considered 
complete. If any item is missing, your application will continue to be deemed incomplete.  If you fail 
to timely submit the requested information, the application will be deemed withdrawn. See 12 V.I.R.R. 
§ 910-6(e).

FORM L&WD-6: 
1) Submit current Tax Clearance Letter from VI Bureau of Internal Revenue for Liberty 
Mobile USVI, Inc.  
Additional Documents 
1) Submit a cross-section profile of the area showing how excavation will affect 
topography.  
2) Submit a list of plant species found at the site or provide any evidence of attempts to 
identify species of interest.  
3) Submit a graphic depicting the coverage area of the tower and any study that provided 
the basis of the decision for the location of the tower.
