AMENDMENT OF



CONTRACT FOR PROFESSIONAL SERVICES



THIS AGREEMENT is made this \ a day of Aas, (i , 20{], in the



Territory. of the Virgin Islands, by and between the GOVERNMENT of the Virgin

Islands, DEPARTMENT OF PROPERTY AND PROCUREMENT, on behalf of the

DEPARTMENT OF HEALTH (hereinafter referred to as "GOVERNMENT") and

REFLECTXION RESOURCES, INC. (hereinafter referred to as "CONTRACTOR").



Both parties witness to being duly authorized to execute this agreement and will

attest accordingly when and where necessary.



WITNESSETH:



WHEREAS, pursuant to Contract No. POO9DOHC17 approved by the Governor

on October 20, 2016 and its Exercise of Renewal Option (No. PO17DOHC17), approved

by the Commissioner of the Department of Property and Procurement on January 13,

2017 (which constitute and are hereinafter referred to as the “Contract”), the

GOVERNEMNT contracted with CONTRACTOR to provide technical assistant in

implementing a Central Cancer Registry at the Department of Health;



WHEREAS, the Contract expires pursuant to its terms on April 20, 2017; and



WHEREAS, the parties desire to extend the term of the Contract to terminate

August 30, 2017; and



WHEREAS, the amendment of the Contract shall not result in any change to the

scope of work provided for under the Contract and shall not require an increase to the

compensation originally provided for under the Contract.



NOW, THEREFORE, for mutual and valuable consideration, the receipt and

sufficiency of which is hereby acknowledged, the parties hereto agree as follows:
