What to bring to the sign-up event: 
1. Picture ID (driver’s license or passport) 
2. Social Security Card (or other official document with your name and SSN) 
3. Copy of voided check for bank account you will use to deposit funds 
4. Proof of physical address (such as utility bill, rental agreement, or purchase 
agreement) 
5. For non-profit organizations – proof of tax exempt status/IRS determination letter
