Please attach the Application Fee or Receipt: $500.00 for Major Permit, $50.00 for Minor Permit. Make Certified Check or Money 
Order payable to the Department of Planning and Natural Resources. 
*Required Supporting Documents: 
a) 
Proof of Legal Interest of all Assignees. (If transfer stems from bankruptcy, then a copy of all papers filed in the bankruptcy 
must be attached). 
b) 
Assignor’s affidavit requesting assignment and transfer. 
c) 
Assignee’s affidavit stating that they have read and are familiar with the permit and will comply with all the terms and 
conditions of the Permit. 
d) 
Tax clearance letter from the Bureau of Internal Revenue 
e) 
Property tax clearance letter from the Lieutenant Governor’s Office. 
f) 
Corporations and Associations: Certificate of Good Standing or equivalent, organizational documents & amendments 
(Articles, Bylaws, Operating Agreement, Declarations, etc.)
