All items noted on this checklist, the drawing requirements handout, and in the application, guidelines must be included or adequately addressed for an application to be considered complete. If any item is missing, your application will continue to be deemed incomplete. If you fail to submit the requested information within 90 days of this letter, pursuant to 12 VIRR § 910- 6, the application will be deemed voluntarily withdrawn by the applicant, unless an extension of time to respond to the deficiencies is requested and approved in writing. Sincerely, Marlon Hibbert Director of CZM LIST OF DEFICIENCIES Development Permit Application/ Drawings-  5.10 b Critical areas and possible trouble spots Please indicate on the site plan where the gut that runs through the property is located. It is mentioned in the narrative of the EAR but no reference is made to its location on the site plan.  6.09 Rare and Endangered Species Please provide any documentation to support the statements made in the EAR regarding rare and endangered species. If none is available, please have a survey done to capture the species present on the potential impact site.  7.0 b Sewage Treatment and Disposal Having reviewed the document, we have a concern, does the present system proposed take into consideration the inputs from the animal kennels, and if it does not, what is the proposed method of disposal of the waste from the animals housed at the center.  11.00 OTHER REQUIRED SUBMITTALS o Please provide an updated resolution with the current board of Directors. The document provided was dated 2017/2018. o Additionally, please provide a resolution stating that Denise Walker is authorized to sign on behalf of the Applicant.  Zoning Concern o Under Act 8727 it was required by the applicant to conduct a noise survey, this should have been included as a part of the EAR submission to the Department. Please explain if that task is being undertaken at this time and when it will be completed. o The zoning variance also required a design for a soundproof building, based on review of the drawings submitted, it is noted that concrete walls are the proposed methodology but with extensive windows, please confirm that those windows proposed are to be sound proofed. Additional Concerns: Please provide a narrative that explains the current statistics for the current location of the animal care center, this should include number of animals being treated yearly, number of spaces available for housing animals and current ownership of building that is currently being used as the center. Please remember to address all the applicable items listed on the Major Land Development Permit Application checklist.
